<p>Features of Botulinum Toxin of A type were accidentally revealed by american
ophtalmologist Alan Scott in 1970, after which other scientists started
developing and testing medications based on it. It was being introduced in
medical practice and cosmetology.</p>
<p>Facial skin is directly connected with mimic muscles, because unlike skeleton
muscles, they don’t have double fixture on bones, and one or both ends are
interconnected with skin and mucous membranes. When these muscles contract they
make skin move. When they relax, skin restores its normal state due to its
elasticity. Mimic movements form wrinkles on the forehead, bridge of nose and
around eyes. If mimic muscles are blocked, the skin around them restores its
elasticity, and wrinkles are evened.</p>
<p>Botulinum Toxin blocks transmission of motor impulse from the nerve to the
muscles. When the substance is injected, mimic muscles relax. However, it does
not cause muscular dystrophy, and blood supply volume stays the same.
Long-lasting relaxation of contracted mimic muscles contributes to evening of
mimic wrinkles, but a person can get used to controlling facial expression. In
4-6 months, botulinum toxin is totally removed from the body, and its effect
disappears — muscles regain the ability to contract.</p>
<h3 id="botulism-and-botox-is-there-anything-in-common">Botulism and botox: is there anything in common?</h3>
<p><em>Botulism</em> — is a dangerous toxico-infectional disease that develops when
Clostridium botulini bacteria get into human body with contaminated foods. It
affects the nervous system and may lead to fatal outcomes.</p>
<p><em>Botox</em> — is a product that contains botulinum toxin, and it’s impossible
to get intoxicated with it because of its minor concentration. It is injected
locally in the muscles that should be relaxed, but after 4-6-9 months this
neurotoxin is fully removed from the body.</p>
<p>As you can see, the only thing in common is the nature of toxin and effect of
muscle relaxation.</p>
<h3 id="should-i-be-afraid-of-this-procedure">Should I be afraid of this procedure?</h3>
<p>Some people are scared of being contaminated with the toxin injected —
it’s a normal reaction. But it’s crucial to understand the difference between
disease and positive features of weakened toxin that is used in medicine. Get

<p>In some cases, the effect is visible several hours after the procedure, but
usually it reveals on the 7th day, and is fully visible in 10 days. 2 weeks
after the procedure, a person should visit the doctor: he will evaluate the
quality of injections and, if necessary, correct the zones where more Botox is
required.</p>
<p>The effect of Botulinum therapy lasts for 4-6 months, but in some patients it
may preserve for 6 months without additional correction. In 6 months, the
procedure is repeated again.</p>
<p>Generally, patients need lower amount of Botox for preceding procedures, because
facial muscles “lose the ability” to contract by habit, and the skin in the area
of injections becomes more elastic, metabolism and trophism exchange improves,
collagen and elastin are produced in higher amounts, which make face look fresh
and smooth.</p>
<p>Botulinum therapy is recommended to be made 1-2 times a year when the neurotoxin
is removed, but not more often than 4 times a year.</p>

<p>All above mentioned complications after the procedure pass with the time being,
and depend on indications, contraindications and the way person follows doctor’s
recommendations.</p>
<h3 id="how-to-get-rid-of-excessive-sweating-with-the-help-of-botulinum-toxin-therapy">How to get rid of excessive sweating with the help of botulinum toxin therapy?</h3>
<p>Botulinum toxin blocks the impulse sent not to muscles only, but to perspiratory
glands, as well, which reduces sweating. After Botox injections, sweating stops
in 2-3 days. The effect lasts for 6-12 months. It does not affect normal
thermoregulation, and the product improves overall skin health: it is optimally
moisturized.</p>
<p>Since injections in palms, axillary creases and feet are pretty painful,
anesthetic cream is applied on it beforehand. The recommendations are the same

purification, storage conditions, dosage and rarefaction, extent of diffusion in
tissues, duration and strength of the effect. The medication and its dosage is
figured out individually based on patient’s indications, resistance and
sensitivity to products.</p>
<h3 id="what-botulinum-toxin-therapy-is-combined-with">What Botulinum toxin therapy is combined with?</h3>
<p>Simultaneously, a doctor can perform threadlifting (installation of
mesothreads), contour plastics with fillers based on hyaluronic acid in the zone
of nasal labial folds, cheekbones and lips.</p>
<p>Mesotherapy, biorevitalization and bioreparation should better be performed in
1-2 weeks, because the procedures boost blood flow in the zones of injection,

From aesthetic standpoint, nose is the central part of person’s face. It’s also
important from functional standpoint: nose is responsible for breathing and
olfaction.</p>
<p>Rhinoplasty is a modern operation that’s performed to restore normal breathing
function and eliminate aesthetic defects of the exterior part of the nose (its
form is changed).</p>
<p>Rhinoplasty also helps to eliminate inborn defects and acquired defects of
osteochondrous part of the nose. Pretty often, surgeons also perform septoplasty
during rhinoplasty — they make plastics of deflected nasal septum.
Deflected nasal septum can either be inborn, or acquired, and may cause problems
with breathing, allergic reactions (allergic rhinitis) from the side of mucous
membrane.</p>
<p>Indications for such operation can be the following: aesthetic defects of the
nose (crook in the nose, low nasal bridge, a long nose, deformation of nose end)
and functional defects (problems breathing, sleep apnea).</p>
<p>Today, rhinoplasty is a complicated, high-precision operation where every
millimeter should be measured, and its positive result depends on surgeon’s
professionalism and artificial approach. Keep in mind that every rhinoplasty
operation in one and the same patient poses challenges for the next surgeon
performing them. Sometimes patient doesn’t understand how exactly the surgery
was performed, so the surgeon should foresee all possible variants, plan every
stage and step of the operation, etc.</p>
<p>Rhinoplastics can be performed with open or close approaches. When closed
rhinoplasty is made, cuts are made in the mucous membrane: such operation is
usually made with the help of an endoscope, and it causes less bleeding while
requiring shorter recovery period. Open rhinoplasty is made with a cut on the
skin (columella), soft tissues are separated from nasal bones and cartilage, and
cartilaginous structures are modeled and installed in the right position. The
choice of method depends on the initial state of patient’s nose, previous
plastic surgeries and surgeon’s preferences.</p>
<p>On the primary consultation, a surgeon performs visual examination, gathers
patient’s medical history (whether any other operations were performed before,
checks for chronic diseases of nose and accessory sinuses of the nose,
peculiarities of injuries and traumas), makes images of the nose, performs
computer tomography of facial bones, and plans a special treatment program
together with visualization. An anesthesiologist also organizes a consultation,
checks the results of analyses and gives his consent for the operation. The
duration of operation depends on its complexity and can take from 2 to 4 hours.</p>
<p>Contraindications are the following: age up to 18 (except traumatic deformations
of the nose), chronic diseases of ear, nose, or throat in acute stages,
cold-related diseases, empyesis and herpetic fever on the skin near nose,
diabetes, poor blood coagulability, oncologic diseases, and psychological
disorders.</p>
<p>During rehabilitation period, a patient has to wear a plaster bandage on the
nose for about 2 weeks — it fixes nose form and prevents swelling. After
the operation, breathing restores, although a person may have hemotomas in
infraorbital regions (caused by osteotomy of nose bones), swelling in the middle
part of face that will disappear by the end of the second week after the
operation. Preliminary results of rhinoplastics can be visible in 10 days. Final
aesthetic result of rhinoplastics cam be evaluated in 6 months, or even in a
year when the changes of osteohondrous structures and soft tissues will finally
fix. During 2 months after the operation, a person is prohibited to do sports,
go through thermal procedures (sauna, bathhouse), smoking and drinking are also
not recommended. It’s crucial to visit doctor regularly for being examined and

<p>It is sad to admit that for many years the oncology statistics has been rather
grim for the international community. I believe each of us knows what cancer is.
Someone knows a person who struggled with this cruel disease, others heard the
bad news themselves. It is a sad beginning. I know. But such things need to be
discussed. It is better to be aware of them. Forewarned, forearmed. Right? By
covering various topics in such areas as dentistry, cosmetology and maxillofacial
surgery under the guidance of doctor-surgeon Alexander Antipov, we have addressed
many of the issues and unknowingly ignored this one. No doubt, you will understand
why. But still, the today's agenda is cancer of the oral cavity (oral cancer).</p>
<h2 id="how-serious-is-the-problem-of-oral-cancer">How serious is the problem of oral cancer?</h2>
<p>When I researched the topic, Doctor Antipov recommended to search online the
statistical data of the World Health Organization. I cannot say that I was
shocked by the fact that oncology is widespread throughout the world. But when I
learned that about 10 million people annually are diagnosed with this disastrous
disease, I came to a standstill, thinking about this number. This means that
approximately 2,700 people a day throughout the entire world hear the diagnosis
— cancer. There is something to think about, isn't it?</p>
<h2 id="interview-with-dr-alexander-antipov">Interview with Dr. Alexander Antipov</h2>
<p>"Of course, oral cancer is not so widespread, but it happens, which means that
it cannot be ignored," Alexander says. The disease responds to treatment, most
tumors can be easily removed, including by means of surgery. If tumors are
nonresectable, they are treated with chemo-and radiotherapy. It is clear that
the sooner you are diagnosed, the better are your chances to recover.
"The disease can be identified easily, even at the initial stage. When inspecting
the mouth in the mirror, you can detect the symptoms, if there are any. When you
visit a dentist, the doctor will also see neoplasms or other signs of oncology,"
says doctor Antipov.</p>
<p>So…, the symptoms and premonitory signs of cancer include the following:
indurations, swelling, red-and-white spots, ulcerations, idiopathic numbness or
bleeding of the gums, pain in any part of the cheek, neck, or mouth. If you have
difficulty swallowing, chewing, your voice is changed or becomes hoarse, this
might suggest the disease. As we often heard at the lectures - it is not required
to write the information down, but it is important to remember it.</p>
<p>For those, who are particularly health-conscious, I will mention at once that
the above symptoms can sometimes result from the simplest actions or the EENT
diseases. So, there is no need to panic ahead of the game. However if anything
bothers you persistently or for a long time, it is best to make an appointment
with a specialist.</p>
<p>"The doctor should inspect the oral cavity, neck, cheeks, and lymph nodes. If
there are any alarming signs, the next step will be the following tests: MRI,
NMR, and biopsy. An ear-nose-throat specialist or a dentist will advise on tests
and procedures to determine a complete diagnosis. Thus, an oncologist can join
the treatment," Dr. Antipov explains.</p>
<p>Despite endless medical researches, there are no apparent reasons why some people
develop cancer and specifically oral cancer. However, it is known that smoking
and the use of chewing or sniffing tobacco, as well as alcohol increase the risk
of cancer. In addition to the above factors, the possible reasons may also
include daily work with hazardous substances, severe lack of vitamin A, constant
injuries of the mucosa, e.g. when you use dentures or dental crowns. I hope that
you all know the dangers of direct sunlight.</p>
<p>"The disease is very rarely found in people under 40. The highest morbidity is
among people 60-70 years of age," Alexander Antipov says.</p>
<p>Is there any way to protect ourselves from malignant tumors? No matter how trite
it sounds, effective ways in this case are a healthy lifestyle and proper
nutrition. Try to avoid alcohol, smoking, eat fruits, vegetables, vitamins, go
in for sports – these are not a panacea, but rather a necessity. As the saying
goes, an ounce of prevention is worth a pound of cure.</p>

me?!" This is especially true of impacted teeth. In part, this view is justified
due to availability of all kinds of videos and information on internet with hours
long and complex operations of removal of wisdom teeth, that tell about scary
consequences, etc. At the same time they completely forget that the wisdom teeth
of those who have them sooner or later will lead to problems so complicated that
the preventive removal will seem a mere trifle. Well, it's the same as to be
afraid of a little mole removal and wait for it to become a major malignant
tumor. Do you need such risks? Read about them here. Well, if I tell you that
the removal of wisdom teeth now takes 15-20 minutes (even in the most complex
cases), is completely painless and safe and at the same time you are, in fact,
preventively solve a number of serious problems — will it change your

<p>Dr. Jackson has a sub-specialty of corrective jaw surgery i.e. ~ orthognathic surgery which he has been performing for the past 30 years. Following his residency training, he spent a year in orthognathic surgery fellowship at St. Mary's Hospital and then did an additional 5 years in the practice doing orthognathic jaw surgery in St. Louis Missouri. He is a member and lectures at the prestigious Arnett Orthognathic Surgery Forum.</p>

<h3> Where is the surgery performed and how long will I be in the hospital? </h3>

<p>Mercy General Hospital or Sutter General Hospital depending on your insurance</p>

<p>you are admitted the morning of surgery and discharged 1-2 days later after you have met the discharge criteria</p>
</li>
</ul>

<h3> How long is recovery? </h3>
<ul>
<li>You will need to be out of work for at least 10 days to two weeks and possibly longer if you have a job that relies heavily on speaking. It is possible that you will need to be out longer, depending on the extent of the surgery</li>
</ul>
</li>
<li>
<h3> Is the surgery painful? </h3>
<ul>
<li>
<p>as a rule, patients do not complain of pain but rather discomfort related to swelling</p>
</li>
<li>
<p>pain medications address this discomfort adequately</p>
</li>
<li>
<p>pain medications are usually not needed after 5 to 7 days</p>
</li>
</ul>
</li>
<li>
<h3> How much swelling occurs with the surgery? </h3>
<ul>
<li>
<p>swelling, extent, size and location is variable</p>
</li>

<p>80% of the swelling is gone within 8-10 weeks</p>
</li>

<p>total resolution of unnoticeable residual swelling may take 12-18 months</p>

<h3> Will my jaws be wired shut? </h3>

<p>light rubber bands between your upper and lower braces are used</p>

<p>orthodontic anchors will be utilized in addition to your orthodontic braces</p>

<h3> How and what can I eat? </h3>
<ul>
<li>
<p>There are four distinct dietary phases which begin with full liquid diet, non-chewing diet, soft foods, and then return to normal diet. You can expect to loose some weight (up to 10% of your body weight), mostly in the first two weeks of surgery. We will monitor your weight after surgery to make sure that you do not lose too much weight</p>
</li>
<li>
<p>Diet Timeline:</p>
<ul>
<li>
<p>first 2 weeks is full time elastics. Removal of elastics permitted only in office. Diet is full liquid, particle free as in Ensure, Boost or particle free Jamba Juice.</p>
</li>
<li>
<p>next 6 weeks with full time elastics, you will be able to remove the elastics three times per day for diet, exercise, and hygiene. Diet is non-chewing which means any food that you can squash with your tongue and swallow without chewing.</p>
</li>
<li>
<p>at 3 months, you will have functional elastics which means you can speak and talk with the elastics on and your diet is soft</p>
</li>
<li>
<p>at 4 months, you may return to normal diet and exercise</p>
</li>
</ul>
</li>
</ul>
</li>
<li>
<h3> Will I be able to exercise after surgery? </h3>
<ul>
<li>
<p>low impact cardio exercise can start 4 weeks after surgery</p>
</li>
<li>
<p>high impact (running etc) and weight lifting can start at 8 weeks (no clenching)</p>
</li>
<li>
<p>contact sports or activities only after 6 months</p>
</li>
<li>
<p>be sure to obtain clearance from the doctor for any change in activity level</p>
</li>

<h3> When can I brush my teeth after surgery? </h3>
<ul>
<li>The goal is to keep you well-nourished and hydrated. In addition, maintaining adequate oral hygiene is equally as important. A Sonicare tooth brush and Water Pick is highly recommended for postoperative use after surgery.</li>
</ul>

<li>
<h3> How soon after surgery can I become pregnant? </h3>
</li>
</ol>
<p>– female patients should wait 12 months after surgery to become pregnant</p>

<h3> Do I have to quite smoking before surgery? </h3>
<ul>
<li>
<p>YES!</p>
</li>
<li>
<p>you must quite smoking at least 2 months before surgery. If you do not quit with ample time, we will have to cancel your surgery date, as smoking prevents healing and encourages bone infection</p>
</li>
</ul>
</li>
<li>
<h3> Are there any other restrictions after surgery? </h3>
<ul>
<li>
<p>you may not play a reed instrument until 12 months after surgery</p>
</li>
<li>
<p>scuba diving and snorkeling may start at 12 months</p>
</li>
<li>
<p>most sport mouth guards should not be worn until 1 year postoperatively, please check with your surgeon before doing so</p>
</li>
<li>
<p>check with Doctor before you wear any type of helmet. Chin pressure from the helmet straps can hurt your temporomandibular joint. In most instances, these types of helmets (that fit tightly over the chin) may not be worn for 12 months from surgery</p>
